MAI 'Tobacco Use Penalty' Details
This correspondence is for current or prior year tobacco users, and employees with family members who use tobacco products and are covered under MAI’s health insurance plan. As in previous years, all employees must certify whether or not they use tobacco products.
Effective January 1, 2018 there will be an added penalty for tobacco users or employees with family members who use tobacco products while covered under MAI’s medical insurance plan. The added penalty is $50.00 per month and will be deducted on a semi-monthly basis from payroll at $25 per pay.
If You and/or Your Family Member DID NOT Participate in Smoking Cessation Program
Tobacco users, who have not previously participated in the offered smoking cessation programs for two consecutive years, will have the option to participate in the offered programs or another program of your choice during 2018 in order to avoid the monthly penalty.
If You and/or Your Family Member(s) Previously Participated in Smoking Cessation Program
If you and/or a family member previously participated in the smoking cessation program offered by MAI or another approved program, for two consecutive years and still currently use tobacco products, the penalty will be assessed.
Employees will continue to have the option of participating for a third year in the offered smoking cessation program or another program of choice; however the penalty will still be assessed.
Employees Electing to Participate in the MAI Paid Smoking Cessation Programs
Any employees who elect to participate in the MAI paid smoking cessation programs, but fail to comply with the program requirements, will need to reimburse MAI for the total program fees.
Employees Electing to Participate in an Alternative Smoking Cessation Program
Employees electing an alternate approved program, outside of the programs offered by MAI, will be reimbursed up to $225. All programs must be preapproved prior to starting.
FALSIFYING ANY INFORMATION ON THE OPEN ENROLLMENT FORMS THAT REQUIRE YOUR SIGNATURE COULD LEAD TO GROUNDS FOR TERMINATION.
Details of programs offered by MAI will be announced sometime in January 2017.

A Tobacco Use Penalty premium is linked to MAI’s medical plan options as of January 1, 2018. All employees electing medical coverage must certify whether or not they use tobacco products.
Penalty Premium Waiver Eligibility
The Tobacco Use Penalty premium is $50.00 per month. Following is how to avoid the monthly penalty premium:
If you are a non-smoking / non-tobacco user, you must confirm so by signing this Tobacco Usage Affidavit.
If you are a Tobacco User, you must participate in the Be Well Solutions Smoking Cessation program or another pre-approved smoking cessation program during 2018. (This Be Well Solutions program may be offered onsite or by other means during the 2018 wellness year. Details will be announced in January.)
Tobacco is defined as any form of the tobacco plant, consumed for the effects of its addictive nicotine properties, that is not prescribed by a licensed medical professional. This term is applied to tobacco/nicotine that can be chewed, dipped, smoked, and/or inhaled (including electronic cigarettes). A tobacco user is defined as an individual who currently uses any form of tobacco, regardless of the method and frequency of use. (Does NOT include Nicotine Replacement Therapy (NRT) products used for quitting.)
